What to expect from a PM&R consultation

A physiatric evaluation looks beyond a single diagnostic label to understand your complete functional picture. Dr. Drake focuses on practical goals that matter to your daily quality of life.

Discussion of your condition & health history

A thorough review of the injury, illness, or diagnosis that brings you to physiatry, including how it began and how it has evolved.

Review of functional limitations & mobility

A detailed conversation about how your condition affects movement, comfort, daily self-care, work, household activities, and independence.

Prior rehabilitation & therapy history

An overview of physical, occupational, or speech therapy you have completed, what helped, and where current functional gaps remain.

Goal setting & next-step planning

Working together to establish clear, realistic priorities for functional recovery, adaptive equipment, therapy adjustments, or medical management.

Physical examination as appropriate

A targeted musculoskeletal or neurologic examination to assess strength, range of motion, muscle tone, sensation, and movement patterns.

Preparation

Helpful items to bring

Having health records, equipment, and your personal questions on hand helps make the most of your consultation time.

Consider bringing the following:

  • Current medication list, including doses and frequency
  • Photo ID and insurance card (if applicable)
  • Relevant imaging reports, operative notes, or hospital records if available
  • Prior physical, occupational, or speech therapy discharge summaries or notes
  • Current orthotics, braces, splints, or prosthetic devices you use
  • Mobility aids you currently use (such as a cane, walker, or wheelchair)
  • A written list of your main functional questions, challenges, and personal goals

NWA Physiatry is currently establishing its operational systems. Updated practice details will be published prior to opening patient scheduling.

Appointment Scheduling

Online appointment scheduling is not yet available. Phone booking will open once practice management systems are finalized.

Referral Requirements

Referral requirements and submission details will be published once scheduling and payer workflows are finalized. Clinicians can review collaborative care information on our Referring Providers page.

Insurance & Payment

Insurance participation and payment policies are being finalized. A list of accepted insurance plans and billing guidance will be published before scheduling begins.

Privacy & Medical Records

Protecting your health information

Please do not submit symptoms, diagnoses, medical records, dates of birth, or insurance information through general website forms or general email.

Clinical intake and medical records will be handled through a dedicated, secure patient portal once systems are live.
